Appointment Changes/ Cancellation
Patients may cancel or reschedule appointments through the patient portal for convenience. We
kindly request at least 4 hours’ notice whenever possible to allow us to accommodate scheduling
needs and offer appointment times to other patients. Missed or no-show appointments may be
subject to a fee to offset the clinical time reserved for your visit.
Patients who arrive more than 15 minutes late may receive a shortened treatment session or may be
required to reschedule their appointment to ensure appropriate care and consideration for all
patients.
Payment Methods
We currently accept credit and debit cards, HSA/FSA cards, and cash (where applicable), as well
as digital payment options offered through the patient portal.
Payment is due at the time services are rendered unless other arrangements have been made in
advance. We are an out-of-network provider; therefore, all applicable deductibles and/or
copayments are the patient’s responsibility and are due at the time of treatment.
Payment and Insurance
Zensation Acupuncture is an out of network provider with all major insurance companies. While we
do not bill insurance directly, we work with many patients who receive reimbursement for their
acupuncture treatments.
If your plan includes acupuncture benefits, we are happy to provide you with a detailed
Superbill. A Superbill is an itemized receipt that includes the standardized medical codes
required for you to submit a claim to your insurance carrier for possible reimbursement.
Depending on your specific policy, reimbursement may be partial or full.
Because we are out of network, payment is due in full at the time of your visit. As part of using
out of network benefits, we do require a card on file for any balances not covered by insurance.
Insurance coverage varies widely. Some plans have deductibles, visit limits, or restrictions on
the conditions that qualify for coverage. We recommend contacting your insurance provider to
confirm your acupuncture benefits and to verify that Superbills are accepted under your policy.
As a courtesy, we are happy to help verify your out of network benefits prior to starting care.

strictly confidential in compliance with applicable privacy laws. We will never
share your information with third parties without your explicit consent, except as required by
law.
Any information submitted through our website contact form is transmitted securely and used
solely for the purpose of responding to your inquiry.
Informed Consent
Before beginning treatment, all patients are required to read and sign an informed
consent form. This document explains the nature of acupuncture treatment, potential
benefits and risks, and your rights as a patient. You will have the opportunity to ask questions
and discuss any concerns before signing.
